Full Time Care Coordinator required for established care provider , rated outstanding by the Care Quality Commission, located in Westcliff-on-Sea, Essex.

The ideal person will be reliable with a good work ethic and work well within an established team maintaining the efficient running of the care coordination department.

THE POSITION

Care Coordination

  • Taking direction from Care Coordination Supervisor and assisting with the following:-
  • management of weekly roster
  • Input and maintenance of client and staff data
  • Oversight of real-time visit monitoring and immediate actions arising
  • Routine checks and controls, to ensure the roster works as well as it can for clients and carers
  • Weekly review visits, including analysis of exceptions using excel spreadsheets
  • Information and reports for management and other departments
  • Forward planning of the roster, highlighting future staffing requirements and issues.
  • Development of roster strategy, procedures and controls

Compliance

  • Maintain accurate records for clients
  • Maintain Health and Safety as required
  • Ensure adherence to DJL policies and procedures

Operations

  • Ensure consistent delivery of quality care using EAISB system
  • Maintain a safe, comfortable and efficient working environment

General office administration

  • Support the reception role by helping with phone calls
  • Data entry and updating spreadsheets
  • paying cash/cheques into the bank

Out of Hours cover (by rota)

  • Visit monitoring, taking immediate actions if required (remote)
  • Emergency phone cover (remote)

Other

  • Liaise and assist other functional areas as required
  • Maintain general procedural records

THE PERSON

  • Keenness and ability to advance within the company and promote the company
  • Excellent communication skills
  • Relevant qualification or commitment to relevant skills training and professional development
  • Excellent working knowledge of Microsoft Office (Word, Excel, Outlook)
  • Organised , ¬†accurate with excellent attention to detail
  • Self motivated , able to work under pressure and exercise clear judgement
  • Positive approach to collaborative working but able to take initiative and work autonomously
  • Have the ability to learn new systems quickly and effectively
  • Well presented

FURTHER INFORMATION

  • Monday to Friday – 07.00-15.00/10.00-18.00 on rotation
  • 37.5 hours per week
  • Saturday’s on rotation as required as either overtime or a weekday off in lieu
  • 28 days paid holiday, including bank holidays plus an extra day for your birthday !
  • Salary range ¬£18525 to ¬£20000 depending on experience
